Hi Chris,
1 - "Password reset" can be disable within the web.config with the enablePasswordReset="false"
2 - For the rest you need to be creative and skilled with the CSS of that module. With CSS and a bit of jquery you can do wonders. I don't have a be-all and end-all answer for this one.
